All five attached forms must be completed IMMEDIATELY.
EPS EMPLOYMENT AGREEMENT
Complete all items.
EPS PERSONAL INFORMATION SHEET
Complete all items.
EMPLOYMENT ELIGIBILITY VERIFICATION (I-9)
You must fill this form out.
Use your full name.
Use your permanent home address.
Follow the instructions for part 2 carefully.
Sign the form at the top section only.
DO NOT SIGN AT THE LOWER SPACE!
XEROX COPIES
You must attach Xerox copies to the documents you checked in part 2 of the ELIGIBILITY VERIFICATION to the I-9. (The most common documents would be your driver’s license and social security card though any proper combination of the appropriate documents is of course acceptable.)
TAX FORMS
You must use your full name.
Don’t forget to mark marital status or number of deductions.
You must use your permanent home address.
Tax forms will not be accepted with a college address.
DIRECT DEPOSIT FORM
EPS pay days are on the 15th and the last day of each month.
Checks will be mailed on these dates. Please allow 5-7 days for delivery by US mail.
Pay Days will be the previous business day when the pay date falls on a weekend or holiday.
Direct Deposit is available to all who have bank accounts in the United States.
We prefer this method of payment. You will too.
If you elect to not sign up for direct deposit, please note that all checks are mailed on payday.
It may take up to 5-7 business days for the US Postal Service to deliver these checks.
NO SPECIAL ARRANGEMENTS FOR PICKING UP CHECKS WILL BE MADE.
